Required Dental Screening for all 9th Graders

Iowa law requires that any child who is entering ninth grade provide the school with proof of a dental check-up.  The purpose of this law is to improve the oral health of Iowa’s children.  The law became effective on July 1, 2008.

The Certificate of Dental Screening is attached.  A dentist or dental hygienist must complete the form.  This certificate must be used. 

Please turn in the completed dental certificate to SWV High School at the beginning of the 2014-2015 school year or at registration at either school. The Iowa Dept. of Dental Health does audit the certificates at our school.

 If you have questions or need help getting a dental check-up for your child, please contact me and I will be happy to help you.  There is a program called I-Smile and the coordinator in our area can help arrange a check-up if your child does not have a dentist or the cost of an exam is a problem.  I have the information on the I-Smile program.
